Histological and Biomechanical Changes of Transitional Fibrocartilage Zone of Tendon Insertion after Autograft of Achilles Tendon-Bone Unit in Rabbits


Abstract
Transitional fibrocartilage zone of tendon insertion site has been known to play a significant role in transmiting muscle force to rigid bone.
Recently, bone-patellar tendon-bone graft as autograft is widely used in ligament reconstruction of knee.
However, the histological and biomechanical changes of transitional zome were not clarified yet.
The purpose of this study was to investigate the histological and biomechanical changes of transitional zone after Achilles tendone-bone autograft in rabbits.
Thirty eight rabbits, weighing from 2500 gm to 3000 gm, were divided into experimental and control group. Eleven of them were used for control and 27 for experimental group.
On the heel of experimental group, tentomy of Achilles tendon was performed at 3 cm proximal to its insertion on calcaneus and osteotomy was performed at 5 mm distal to insertion site of Achilles tendon.
These detached Achilles tendon-bone units including transitional zone were exchanged between right and left heel.
Transplanted Achilles tendon-bone units were sutured to recipient tendon and tixed to calcaneus with K-wire.
The both operated legs were immobilized with short leg casts for 4 weeks.
For control group, rabbits were immobilized with bilateral short leg casts for 4 weeks.
Afterward, 3 of them were sacrificed for histological study and 8 for biomechanical study.
At postoperative 4, 8 and 16 weeks, nine ribbits(3 for histological study, 6 for biomechanical study) of experimental group were sacrificed.
The harvested specimens for biomechanical study were tested with tensile testing machine and the tissue specimens for histological study were stained with hematoxylin and eosin.
@ES The results were as follows :
@EN 1. In control group, the transitional zone with non-mineralized and mineralized fibrocartilage was observed.
The cellular parallelism in fibrocartilage and basophilic tidemark were observed.
2. In expermental gorup, at post-operative 4 weeks, hypocellularity and loss of cellular parallelism suggesting necrotic phase were observed.
At post-operative 8 weeks, cellular proliferation without parallelism was observed.
At post-operative 16 weeks, cellular parallelism similar to control group suggesting remodelling phase was observed.
3. The mean maximum breaking strength of control group was 98.99¡¾12.44 N. The mean maximum breaking strength of post-operative 4, 8 and 16 weeks groups were 36.46¡¾13.33N, 49.61¡¾10.01 N and 83.07¡¾15.55 N, respectively.
4. The mean tensile stiffness of control gorup was 19.20¡¾3.96.
The mean tensile stiffness of post-operative 4, 8 and 16 weeks groups were 17.53¡¾5.30, 18.08¡¾4.57 and 18.79¡¾2.65, respectively.
It was demonstrated that transitional fibocartilage zone in Achilles tendon-bone unit after autogenous graft regained its original strength with reconstitution of cellular parallelism as time passed.
